To make the most of our time together, please follow the instructions in your booking confirmation email. These instructions include setting up a Patient Profile in Optimantra, our medical information system. You will then be prompted to complete your medical history, including any prescriptions/supplements you take, a detailed questionnaire, and related consent forms. Note that these forms may take 20 minutes to complete, but they are vital to your care and need only be done once.
If possible, please come to your visit knowing your most recent blood pressure.

Yes, continuity is important to us. You are welcome to see the same provider for a follow-up or new concern, and you are also welcome to schedule with a new provider if you prefer.

Follow-ups are essential to our collaborative process. Typically, follow-up times range from four weeks to three months but vary depending on your medical issues and treatments.

Yes. We cannot serve as your primary care practitioner. Additionally, as we offer Telehealth only, we are unable to perform physical exams. Therefore, it is in your best interest to keep your Primary Care and OBGYN practitioners. We are happy to send your practitioner test results or any information you would like us to share.

The deposit helps us secure your time and ensures we can provide dedicated care to every patient. This deposit is applied to the cost of your appointment.
